  • Read Only - a text field where you are to add some survey or survey section description. No answer options here 

 

The types of questions where the survey respondent is to select from the predefined answers:  


  • Single choice question - the respondent is to select only one answer 

  • Multiple choice - the respondent can select one and more answers 

  • Grouped - it’s a group of questions (arranged in lines) that have the same answers which are structured in columns 
          Maximum rate in the group is 15

  • Analogue (so-called scale question) - allows to add questions with scales. There's the ability to define the required “step length” (from 0 to 5; from 1 to 100 and so on) 

The types of questions where the survey respondent is to fill in some type of info: 


  • Text question  

  • Number question

  • Date - the date format is set according to the survey respondent’s time zone settings 

  • File - the ability to add PDF, PNG, GIF, JPEG, HEIC file types. You can hide the uploaded images from your patient (if they are not suitable for showing them

 

Each question type offers the ability to store it in Cognito if the data is sensitive and to be strongly protected.


 

Thus, all the question types can be included in a survey separately (individually) or grouped in bundles and scoring groups for further calculations and graph building.
